Hi, guys —
I received Holy Communion with a mortal sin on my soul, but I had intentions to go to Confession afterwards.
The scary thing is I have done this multiple times, but went to Confession afterwords. I don't do this often, however I feel that this is a serious thing.

You are correct, this is a serious thing. One should always go to Confession before receiving Communion if one is conscious of mortal sin.
Perhaps you didn't fully understand this, and this might mitigate some guilt; but if I were you
I would explain this in your next sacramental Confession to the priest ... then begin to never receive Our Lord in Holy Communion if conscious of mortal sin, which, in itself, separates us from Him.

You feeling is justified by the writings of St. Paul in Corinthians:
26 For as often as ye eat this bread, and drink this cup, ye do shew the Lord's death till he come. 27 Wherefore whosoever shall eat this bread, and drink this cup of the Lord, unworthily, shall be guilty of the body and blood of the Lord. 28 But let a man examine himself, and so let him eat of that bread, and drink of that cup. 29 For he that eateth and drinketh unworthily, eateth and drinketh damnation to himself, not discerning the Lord's body.
1 Corinthians 11:26-29
