I have a Miraculous Medal that I wear with devotion daily and it has everything that it should have on it except for the oval frame on the front side.
- Is it necessary for this medal to be made exactly the way it was originally designed, in order to receive the graces associated with it?

or something equivalent?

Yes, it does have that inscription.
It's just missing the actual oval "frame" around that inscription.
