The short answer: Go to Confession and welcome back!

This, of course, assumes there are no other impediments. Sometimes there are divorce and remarriage issues etc., but it doesn't sound like that's the case. Go talk to a priest and he'll be able to tell you what to do. If he doesn't, because of complicating circumstances, he'll know who to ask.

One last point. It's great that you want to come back, but you should also make an attempt to learn about the faith. You left it pretty easily. It sounds like you didn't know what you leaving.

I'd recommend enrolling in an RCIA (Rite of Christian Initiation for Adults) program. You don't technically need it to resume the sacramental life; you could probably do that by simply going to  Confession. What you could do is enter the program as an observer to learn about the faith and the Church you left.
