Generally, you don't know with certainty. Nevertheless, sometimes an improbable coincidence will happen together with a powerful conviction that it's from the Lord.

This happened to me the other day when I was really struggling with something, wondering why the Lord was not granting fervent, plaintive prayers that I thought were pretty much no-brainers.

I was at a Bible study when we saw a DVD that addressed the very question of persevering in prayer despite lack of any external signs of success, together with a psalm that really spoke to the situation I was going through.

I took this as an encouragement from the Lord and indeed, the next day, the situation I was struggling with, eased dramatically.
