Ann,

This is called the Chaplet of the Blessed Sacrament (Approved on May 30, 1911 by Pope St. Pius X [New Advent][Wikipedia].

How to pray the Chaplet of the Blessed Sacrament:

This Chaplet consists of a medal of the Blessed Sacrament and 33 beads. The 33 beads represent the 33 years of Christ's life on earth.

On the medal, make a Spiritual Communion:

"As I cannot now receive Thee, my Jesus,
in Holy Communion, come,
spiritually into my heart,
and make it Thine own forever."

Or you can use Mike's version:

Spiritual Communion

O Lord Jesus, I believe that You are present in the Most Blessed Sacrament of the Altar.

I love you above all things, (with all my mind, with all my heart, and with all my soul).
I love you because you are infinitely good and worthy of all my love.

Since I cannot receive You now sacramentally, at least come spiritually into my heart.
I embrace myself entirely to You and unite myself wholly to You.
Never permit me to be separated from You.

Come Lord Jesus and glorify Yourself through my weak, broken body.

Amen.


On each bead, say:

"Jesus in the Blessed Sacrament on the altar, have mercy on us!"

Based on images of the Chaplet of the Blessed Sacrament I found on the web, I don't think the image was that of St. Vincent De Paul.

A google search gave me these results:

On the obverse of the medal is an image of Jesus holding the Blessed Sacrament, a chalice, and a child, with the Latin words Panis Vitae, Bread of Life.
